The City Primary Care Liaison Team (Learning Disabilities) is a team of learning disability nurses who work across Primary Care. The team provides support to adults with learning disabilities, carers and professionals, to understand, maintain and improve their physical and mental health needs, whilst also reducing identified health inequalities.
